What happened here? Was the stand on especially steep slope? The video is quite blurry, but did the marshaller clear to start engines before the pushback truck was properly attached to front gear? Just a thought, but can starting one engine cause such a rapid movement forwards as seen on the video? As a trivia, two engines can be started at the same time on the 744.

Definitely an "oops".

Please remain seated with your seat belts fastened until captain Crash has finished slamming us into the terminal.

SAA plane knocks over a container lorry! What are the chances of capturing these incidents on camera?

Pretty good actually, everyone has video cameras and many people have cell phone cameras, so cameras are everywhere.

No parking brake, I guess.

In my update for T.Ruths A306/310 you can't start the engines on the ground unless you've got the parking brake set AND at least one hydraulic system on to power them.
Just to avert those nasty "kiss the terminal" incidents.

So it comes down to two most probable possibilities. A) Crew skipped checking parking brake during checklist for some reason B) Ground crew mishap. I still do believe is this possibility, as two ground handlers are moving a tow bar (?) as they flee from path of the aircraft.

Inertia of a 747 is so great that you probably won´t notice movement without outside visual reference, as crew´s eyes are on EICAS during start-up.
