Missions vs Improving Realism. Or Both?

How realistic do you fly? Or do you concentrate on missions and campaigns, keeping the flying less complex?
I'm considering going more realistic, but not really sure where to started on flying with more realism. Any thoughts?
Or does that take away from the missions and campaigns (especially for more casual use)?

Although I have no work to do, so I suppose I could spend alot more time on this...

i usually fly with complex enginge management 'off' and realistic gunnery 'off'.

realistic gunnery really hampers your ability to shoot down anything with one pass unless you are just extremely lucky...it also doesn't seem to give the same 'realism' to the AI which can kill you with ease...
