Sounds like an odd one. I've seen a number of jumpers that get complacent with their equipment after they jump for a while. But 1K jumps plus you would think they would be over that stage.

May have had health issues and his chute failed? Hmmmmm... hate to say it, but he might have purposely rigged his chute to fail.
