The US Navy has this aviation safety magazine called “Approach”. The idea there is that you tell your harrowing tales of doing stupid things, in the hope that I might learn from your experience without having to do the same stupid thing myself first. There are a lot of amazing stories there. One in particular I remember because the story seemed to match so well the video in this thread. The difference was that all four souls on the plane were very experienced naval aviators, suffering from “get-there-itis”, the affliction that causes pilots to do dumb things because they don’t want to divert or not go in the first place. Interesting video.
That makes a lot of sense PRB.
For many years in europe, it has been the policy of aviation safety boards, to find caurses, not blames, when incidents or accidents occur.
It certainly makes the pilots or ground crew much more cooperative, when an investigation is going on.
Of course it can get so irresponsible, that some legal action has to be taken later on.
But the main reason for investigations should not be to "find somebody to blame"
