And yeah, this is pretty messed up. Some of these programs should be well-clear of the Explody phase.
 
The very principle of a rocket engine is the barely constrained and directed output of an explosive chemical reaction. One mechanical failure either through design or manufacturing error takes away the constraint element and leaves you with just the explosion part! That's rocket engine design 101 right there in a nutshell.
