Only one I know of I downloaded from Avsimrus.com. Type yak-12 in the search box then it's the second result in the list "Yak-12 aircraft final version". Also quite a few repaints and I think a file with different cockpit textures.
It is a port though, originally for FS2004. Haven't flown this one in awhile but don't remember there being any major problems.
